NOTE: Although this is a position with Roaring Fork Schools (Re-1), this position is located west of Glenwood Springs, in Rifle, CO, and is embedded within the Garfield Re-2 School District.
DEFINITION: Works in the Garfield School District RE2 schools to support students in developing the enduring knowledge, skills and character to thrive in a changing world by providing case management, service linkages, and community supports their families through the Colorado Community Response Program, (CCR).

Vision: A powerful partnership with vulnerable families to proactively increase protective capacities and improve child safety in Colorado.
Mission: To provide a comprehensive, community-based service continuum for families at-risk for child maltreatment through a combination of case management, service linkages, and community supports.

PRIMARY DUTIES
- Provide comprehensive voluntary services for RE2 families reported to child protective services but screened out and/or closed after initial assessment
- Increase enrolled families’ protective capacities by promoting individual, family, and community strengths
- Connect enrolled families to vital economic and other support services
- Collaborate with partnering agencies to offer comprehensive services for families
- Coordinate services with other community programs offering services to parent and their children
- Provide strengths-based services to enrolled families through home visitation
- Assess family need and provide appropriate information and support to parents
- Complete all required documentation regarding home visits, attendance records, screening summaries, essential records in a timely and comprehensive manner
- Attend all required trainings under the Community Response Program requirements
- Maintain collaboration and communication with school staff, regarding student and family progress and needs
- Accurately report program participant information into the CDHS data system
- Use only evaluation tools/surveys provided by CDHS
- Adhere to Statement of Work (SOW) and CDHS contract
- Achieve to Program performance targets
QUALIFICATIONS
- Basic understanding of the stages of child development and language development
- Philosophy of strengths-based approach when working with families
- Ability to develop trusting relationships with families
- Ability to develop relationships with a diverse community
- Knowledge of the available community resources
- Ability to take initiative and identify and develop systems within a new program
- Recognize potentially hostile situations and defuse situations appropriately.
- Knowledge of various computer programs and technology
- Ability to collect and manage data while practicing confidentiality
- Principles and procedures for record keeping
- Principles of business and letter writing and basic report preparation
- Basic mathematical principles
- Work efficiently with a service oriented attitude under stressful situations with frequent interruption.
- Work independently in the absence of supervision.
- Effectively manage time and multiple responsibilities.
- Communicate clearly and concisely, both orally and in writing.
- Establish and maintain effective working relationships with those contacted in the course of work.
- Interpret and apply District policies and procedures.
- Independently prepare correspondence and memoranda.
- Implement and maintain standard filing system.
- Bilingual (Spanish and English) preferred
EXPERIENCE & TRAINING GUIDELINES
- 3 years experience with family case management, early childhood services or human services related background
- Ability to obtain and maintain a valid driver’s license.
WORKING CONDITIONS
- Office environment; exposure to computer screens.
- Travel from site to site.
- Flexible hours to accommodate family needs
- Home visitation
- Physical Conditions: This position requires ability to give tours of schools and the community. There will be walking, climbing stairs, and sharing spaces.
- Primary functions may require maintaining physical condition necessary for sitting for prolonged periods of times; use of computer keyboard.
